The City of Courtenay is collecting personal information in accordance with Section 26 (a,b,c,d) of the Freedom of Information and Protection of Privacy Act and will be used for the purposes of conducting an investigation under appropriate City of Courtenay bylaws. Information collected on this form is held in strict confidence except as required in a Court of Law. Should this matter proceed to Court, you may be required to give evidence as a witness and your complaint will become public information.
